New Patients

New Patient Registration

The doctors welcome new patients who live within our practice area.  As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor. 

Medical treatment is available from the date of registration.  Please contact reception for further information.

New Practice Boundary

In consultation with our Patient Participation Group and the North Kirklees Clinical Commissioning Group, our boundary map changed on 1st April 2019.

We no longer accept registrations from patients living in the shaded are shown on the map below. Any patients already registered with us and living in the shaded area will continue to be registered until such a time that they move out of the new boundary area.

Pre-registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the “Send” button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct. Please note that you must attend surgery within one week of completing the online forms or they will be destroyed.

It is important that you also complete the relevant patient registration form below depending on your age, and bring this into the surgery with some form of identification.

Your registration will not be processed until we have both forms and your ID.

It can take some time for us to receive your medical records electronically, so it is important that we have this information at the point of registration.

Note that by sending the form you will be transmitting information about yourself across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
